Welcome to Pathway to Hope Counseling. By engaging with our services, you agree to the following Terms and Conditions. Please read these terms carefully to understand your rights and responsibilities as a client.
1. Service Agreement
- All therapy services provided are confidential and conducted in accordance with professional ethical standards and state regulations.
- Therapy sessions are conducted either in-person or via Telehealth, depending on your preference and availability.
2. Appointments and Cancellations
- Scheduling: Sessions must be scheduled in advance. You will receive confirmation via email or text message.
- Cancellations: A minimum of 24 hours’ notice is required for cancellations or rescheduling. Failure to provide adequate notice will result in a cancellation fee equal to the full session rate.
- No-Show Policy: If you fail to attend a scheduled session without notice, you will be charged the full session fee.
3. Payments
- Payment is due at the time of service unless other arrangements have been made.
- Accepted payment methods include Visa, Mastercard, American Express, and Discover.
- Clients using insurance are responsible for any co-pays, deductibles, or fees not covered by their provider.
4. Confidentiality
- All information shared during sessions is confidential and protected under HIPAA regulations.
- Exceptions to confidentiality include instances where there is a legal obligation to report (e.g., harm to self or others, child abuse, or court orders).
5. Telehealth Services
- Telehealth sessions are conducted via secure, HIPAA-compliant platforms.
- It is the client’s responsibility to ensure a private and secure environment during online sessions.
6. Insurance and Billing
- We accept a range of insurance providers. Clients are responsible for understanding their insurance benefits, including co-pays, deductibles, and coverage limits.
- Pathway to Hope Counseling will assist in processing claims but is not responsible for disputes between clients and insurance companies.
7. Termination of Services
- Either party may terminate the therapeutic relationship at any time. If termination occurs, Pathway to Hope Counseling will provide appropriate referrals for continued care.
8. Changes to Terms
- Pathway to Hope Counseling reserves the right to update these Terms and Conditions at any time. Clients will be notified of significant changes.
